Immunization

Columbia College Chicago is required by law to comply with the guidelines set forth by the College Immunization Code (77 Ill. Admin. Code 694,) which requires all students born on or after January 1, 1957 who enroll for six credit hours of more to provide the college with proof of immunity against certain communicable diseases.

If you do not have a complete immunization record on file with the Office of the Registrar, a registration hold will be placed on your account and you will not be eligible to register for the following semester.

Immunization Requirements

3 tetanus/diphtheria/pertusiss (DPT, DTP, Td, DTaP, or Tdap) boosters. One must be Tdap.  One must be within the last 10 years.

  • PLEASE NOTE: Tetanus Toxoid (TT) does not meet this requirement.


2 measles, mumps, rubella (MMR) vaccines given after one year of age and after December 31, 1967.

  • OR if measles, mumps, or rubella vaccines were given separately:
    • 2 measles immunizations given after one year of age and after December 31, 1967
    • 2 mumps immunizations after one year of age.
    • 2 rubella immunizations after one year of age.
  • OR a positive blood titer test result for measles, mumps, and rubella
    • If you were previously vaccinated, or were exposed to the disease and believe that you now have immunity, and are unable to provide immunization records, a positive titer result is acceptable as proof of immunization. A copy of the lab report must be submitted.

1 meningococcal conjugate vaccine for students who enter Columbia in Fall 2016 or later, and are age 21 or younger. Must be taken on or after 16th birthday.

  • PLEASE NOTE: Menomune and Meningitis B do not meet this requirement.
How to submit your immunization records

Students starting at Columbia in Summer 2017 and later

Columbia College Chicago is partnering with Med+Proctor to submit your immunization records.

  • Check your official Columbia student email account.  All new students who have confirmed their intention to enroll at Columbia are given a student email account (loop.colum.edu).  You must be able to access your student memail account to complete this process. If you are experiencing any difficulty with your Columbia email account, please contact client services at 312-369-7001.
  • Visit https://secure.medproctor.com
  • Click "register" and create an account in MedProctor using your loop.colum.edu email address
  • Complete your MedProctor profile
  • Download and print the Immunization Certificate form
  • Take the Immunization Certificate form to your medical provider for completion
  • Upload an image of the completed Immunization Certificate form to MedProctor

Please Note:

  • Your full name and OASIS number must be clearly written.  
  • Foreign immunization documents must be translated to English.
  • Columbia College Chicago does not release immunization records. We recommend that you keep a copy of any documents you submit.
